First Visit
When you come in for your first visit to our office you will be welcomed by our warm and friendly staff. One of our office staff will ask you to fill out our patient registration forms and she will go over your insurance information with you.
You can fill out our patient registration forms in the office or download them here, fill them out at home and bring them into our office on your first visit:
Office Hours
Our regular office hours are Mon - Fri 9am - 5pm.
Emergency Services
If you have a dental emergency, please call us at 805-351-4933. We will schedule you to be seen very rapidly, usually the same day.
Dental Insurance Card
If you have dental insurance, please also be sure to bring your dental insurance card with you. Please be aware that this is usually a different card than your medical insurance card.
